Requirements:

• A University Degree in Social Work;
• English Language (Syllabus B) and Chinese Language at Grade E or above in HKCEE / Level 2 in HKDSE;
• 3 years' relevant post-qualification experience;
• Registered under Social Workers Registration Ordinance.

Job Duties:

• Responsible for the effective management and operation of the enhanced home and community care services for the elderly;
• Enhance the integration of the service for frail elderly and provide care management to the users;
• Render individual counselling and referral service;
• Provide support and organize related group activities for carers;
• Liaise with paramedical professionals and sub-contractors;
• Supervise staff and conduct staff training programmes;
• Assist in monitoring new service operation and development.

Working Hours: 44 hours per week and on shift when necessary
Working Location: Wong Tai Sin District
